If you do not have a Military, Retiree, or Federal Government ID card and need access to the base, please fill out the attached sheet and send it back to me.
***The purpose of the SSN on the access sheet is for the base Security Forces to do a background check 2 weeks prior to entry on the base. It is Mandatory for ALL personnel over the age of 18 ***
